Partnership Structure
Independent Contractor Status
- Doctors will join as independent contractors, not employees

- 30-day termination notice required from either party
Qualification Requirements

Doctor Responsibilities

- Respond to patient follow-up questions within 24 hours
Operational Responsibilities
- Maintain updated availability calendar in provider app
- Arrive punctually for all scheduled appointments
- Provide at least 48-hour notice for schedule changes
- Bring all necessary equipment and supplies (as specified in procedure guidelines)
- Complete required continuing education modules
- Participate in quarterly quality assurance reviews
